Meet Entry Form 

Fill out the meet entry form below. Be sure not to leave any fields blank. Once you complete and click the "Submit Meet Entries" button below, the entries are sent directly to julia.lamb@ourclub.com. You should receive a confirmation immediately. Always email Julia, before the meet entry deadline, to confirm that your entries were received.

Make checks payable to "TBAY" and forward it to Julia (18050 Hunter's Oak Court, Tampa, FL 33647) before or on the day of the meet. Parents will be responsible for all entry fees even if their child cannot attend a meet and the entry deadline has passed – no exceptions.

Central Branch members should note that Coach Jason determines ALL meet entries (including deck entries) for the swimmers. If you have any concerns or questions regarding this procedure, please contact Jason at jasonbowes@hotmail.com. Parents, please take the earliest opportunity to let Jason know if your child(ren) will not be able to attend a particular meet. Parents will be responsible for all entry fees even if you inform Jason that your child cannot compete after the entry deadline has passed – no exceptions.
